I believe 120 will be unlocked after you encountered all types of chasers.
134 is still triggered by the lonely Urotsuki event. Go to the Underwater Amusement Park, find the area with the columns. Find the column with scrolling text, interact with it. Nothing will happen, so go on and explore the park a bit, then go back. If the text on the column has changed, interact with it again, and you will be teleported to the event.
I believe 140 is unlocked after you got the first ending.

There was so much evidence to back up the theory that the people were in various stages of rabies, plus the fact that .flow backwards is wolf. The hospital areas, the kaibutsu that weren't yet too far gone enough to chase her, the restraints on the beds in the hospital areas (something they used to do to rabies patients back in the day) so from a medical standpoint I felt that her "dream" world was about her descent into the disease and having to watch those around her deteriorating as well. Of course, I could be reading too far into, but I feel like there's way more behind that game than just a series of creepy visuals.

Wataru appears to have attempted to make the entire area freely explorable; ie, you can reach almost any map in this area of the game from inside the game; ie, there shouldn't be any dead ends and you would be able to visit every single map in one dream session (with out bombing), if you were smart about it. However, this is not 100% the case; there are a couple of programming bugs that render some doors unusable or unreliable. There are also some unfinished areas; in the files it is abundantly clear that Carnival Town will eventually lead back to the Overgrown City; it just hasn't been fully implemented yet.
There are only two areas which have a concrete order, for the time being. One is the Prolog, or Forest Pier. Entering the area this way, the easiest way and probable way wataru intended the player to enter, since a) it's called "Prolog" and b) when you unlock the shortcut to wataru's area this is the place it dumps you out, you see this map first. It is one way, so there's no way back to it from inside wataru's area. So it's the "first". The other area that probably is intended to be the final area is the bottom of the Depths, called "To the Final Place" in the files.
Essentially, once all the doors are implemented and the area is finished, the exploration order will probably be:

found a rather amusing glitch
in the hot spring, if i equip an effect and then unequip it, urotsuki changes into her regular outfit. she changes back into the swimsuit if she sits down.
i guess it messed something up because afterward every time i made urotsuki sit on the ground she'd change into her swimsuit, regardless of location, even outside of the dream world.
it's easily fixed by reentering the hot spring and leaving again, but not being one to waste an opportunity, i took some screenshots first.
