Maybe.

At some point, probably.
Part of me thinks that Madotsuki went to the dream world to escape reality (And be happy in the dream world). But it seems like she went through a lot of work in the dream world just to kill herself.
Although it's hard to tell if she enjoyed the dreams or not. What's sure is that whether it was because of something in the real world or the dream world it wasn't worth it in the end.

I doubt it, just given her lifestyle. As we don't see the rest of her life, we have to assume she's a shut-in, and serious problems are required to do that to people.

She was afraid.

Everybody was a nonchalant fetus once, so I guess yes.

If she had been happy before, it could quite be a reason for why she lived as she did, and eventually committed suicide, as the impossibility of returning back to a happy phase of our pasts, and feeling unable of achieving happiness again, can destroy someone.

I just thought – is it confirmed via code reverse engineering or something that there’s only one ending? Or maybe there’s some path, I mean things to do and things not to do to make Madotsuki happy? Not stabbing anyone, for example?

The game has been popular since 2007, if 8 years of gutting the game in RM and constant playing didn't show anything new, then no.
