I'm pretty sure they've been discussed plenty, may be there's still some stuff left in the archives. I've vague ideas of what their personalities might be like, and that's about it.
I think Masada might've been a piano teacher, somebody kind and artistic but probably not too bright, considering his derpy expression. Toriningen were probably the popular "girl posse" of Madotsuki's school. The ones that chase you were probably the snide, mean girls, and the ones that only chase you when you hit them were probably more indifferent to Mado, but would bully her if they had to. I think Monoe might be hiding something under that smile of hers, something that'd make her vulnerable or something bad that she did, but I don't know what. This is very common, but Monoko probably was hit by a car, but I can't really figure out what she might've been in her life, same for Shitai. Not sure at all about Poniko; I know a lot of people like to depict her and Mado as BFF's but I don't really see it, may be I'm just missing something.

while our minds are not able to create new people we've never met before in our lives, from zero source material from reality, in our dreams… our dreams can mixmatch any kinds of traits we've encountered before to create "new" people…
… and really that is what i always thought… i never really thought of them as having any sort of connection to the outside world, just interconnections in her imagination.
everything you interact with has the potential to be a person from reality though… all the way down to the flowers with which (whom?) she interacts at the top of that cliffside.

I've always thought of the Toriningen as a fucked up manifestation of Madotsuki's mother. They look very tall in comparison to Madotsuki and most of the sprites in the game. I think the height represents Madotsuki's feelings towards each of the individual people in her life. Like Masada, and the Toriningen are tall because they were important in her life. Not to mention that they look somewhat similar to Madotsuki. Like relatives.
The Toriningen is very hard to escape in the game. That's how mothers are. You can't really escape them, and they'll always be in your life in someway. Maybe I'm just overthinking it, and the creator just wanted them to be a hard-to-beat enemy…
The Toriningen are psychotic. I think that Madotsuki's mother was abusive, and just generally a bad mother (this is self explanatory. The toriningen try to kill Madotsuki and send her to inescapable places. Then the player has to wake up Madotsuki. Maybe it represents how her mother "shattered her dreams"?) which is why they are Madotsuki's enemy in the game. It represents Madotsuki's resentful feelings towards her mother. They can also be benign, which might represent how a child feels towards their mother. Despite all the terrible things a mother can do to their child, the child will always feel some sort of appreciation or love towards them, especially young children (maybe she was abused at a young age). Their different personalities may represent Madotsuki's mixed feelings towards her mom.
Or maybe you can take this in a more literal sense. Her mom could have had a disorder of some sort. Say, bi-polar disorder, and Madotsuki inherited it. The Toriningen send her to inescapable areas, causing the player to wake Madotsuki up. This could represent how through the disorder she inherited, she was sent into hopelessness and depression and the depression shattered her dreams. Maybe you could think of the Toriningen as critical parental figures. There are endless possibilities!
I never thought of Monoko and Monoe ever being close to Madotsuki. I think they were just classmates in school, acquaintances or maybe even strangers. Monoe and Monoko are sisters (duh). I think that Monoko and Shitai-san was killed in a car accident that Madotsuki and her mom was involved in. That would explain why the stoplight effect causes Monoko to turn deformed(mutilated by accident), and that thingy with the shaking jaw's jaw to stop moving, like it died. Because of this, Monoe felt resentful towards Madotsuki and began bullying her. That's why she has a smug look on her face during her full screen event. Also, you have to kill Dali/Dave Spector before meeting Monoe. Maybe Dali represents a school staff, or teacher, and the tunnel represents a classroom (due to Madotsuki's depression school was dismal and boring). I don't think that a school staff member was literally killed, but this may represent how the staff members just turned a blind eye to Monoe's bullying.

About Monoko. I always thought that yes, there was a car crash, but it happened when the car was at a stoplight. (herpaderp) Monoko was inside of a car, and she slammed her head on the dashboard (probably not buckled up) Madotsuki had seen her every once in a while, and didnt really remember much of her features, so monoko was colourless, and probably had a few features that were from her imagination so she actually had a full face. One day, Madotsuki saw a vase of flowers on her desk, and tried to remember her face.

Shitai, I beleive was a similar incident (carcrash) but he died outside of a car. Shitai didnt notice the green light, and walked across the street, but got hit and madotsuki witnessed this event, riding on her bicycle in the bicycle lane.

I see Masada and Mars-san as siblings, I don't know why. I just can't really see them being a couple, but I see them being close. Even though Mars-san appears to be much bigger, I imagine her as a little sister.

Whatever happened to Madotsuki happened in fall and winter, which is why you get a rain and snow effect.
Whatever happened in the car crash involved a bike somehow. Modoko was on a bike or Madotsuki.
